Pericoronitis, what is it?
Pericoronitis is an inflammation of the tissues that surround a wisdom tooth during its eruption period.
The causes of pericoronitis are not fully understood. According to one theory, improper tooth eruption is genetically determined, meaning that the anomaly develops in utero. According to another hypothesis, the pathology is associated with changes in the structure of the dental system over the course of life (the width of the dental arch decreases by 10 mm), and the wisdom tooth simply does not have enough space.
During eruption, the gums and adjacent soft tissues are traumatized. When chewing food, the area around the tooth becomes inflamed, which causes pain. The process of eruption slows down and sometimes stops. The situation is further complicated by dense bone tissue in the area of eruption and thickened gums.
In the area of the erupting wisdom tooth, a gum pocket is formed, which provides a favorable environment for the multiplication of pathogenic bacteria. That is why wisdom teeth often erupt with caries and even pulpitis.
Symptoms of pericoronitis
Symptoms of pericoronitis depend on the stage of inflammation. Medical statistics show that inflammatory processes are often preceded by ENT infections: sinusitis, rhinitis, otitis.
In the initial stage, a dull pain occurs in the area where the tooth is erupting, and over time, the intensity of the pain syndrome increases. Opening the mouth and chewing food becomes a real problem.
In the absence of medical help, swelling forms in the gum area, which in turn causes asymmetry of the face oval. As for the general state of health, it remains satisfactory. Sometimes the temperature rises to subfebrile levels, and headaches may appear. In the future, the process becomes chronic, and the severity of symptoms is dulled.
However, the inflammatory process in periocoronitis continues to progress in the soft tissues surrounding the tooth, and spreads to the regional lymph nodes. The formation of serous or purulent exudate causes a putrid odor from the mouth. In the most unfavorable situations, the root of the adjacent tooth may be affected. As the infection spreads (and in the absence of dental care), atrophy of the jawbone and chronic edema of the soft tissues develop.
Classification of pericoronitis
Pericoronitis is classified by the nature of its course (acute and chronic) and by the type of inflammatory process (catarrhal, purulent, ulcerative).
The catarrhal form is the initial stage of the pathology, in which the overhanging edge of the gum becomes inflamed, hyperemic, and painful. At this stage, no exudate is formed, and pain does not occur when the mouth is opened. When receiving dental care, all symptoms quickly disappear.
The ulcerative form develops against the background of persistent inflammation, when soft tissues begin to break down. Small ulcers covered with a grayish plaque form around the tooth on the mucous membrane. Attempts to remove the plaque are accompanied by pain.
The purulent form is a severe form of pericoronitis. The temperature rises to 38 degrees, and overall weakness increases. The patient loses appetite, and there is increased pain when opening the mouth. Pus is released from the area around the tooth, the gums swell, and the lymph nodes enlarge.
A separate form is distinguished for the pathological process that develops behind the tooth (retromolar pericoronitis). The peculiar feature of the disease is that the area of inflammation is located below the tooth crown.
Complications of pericoronitis
Improper eruption of a wisdom tooth against the background of weakened immunity and the presence of chronic infections can lead to a number of complications:
- ulcerative gingivitis;
- periostitis;
- abscess, phlegmon;
- osteomyelitis.
Periostitis and osteomyelitis are the most severe complications, as not only soft tissues but also the periosteum, jawbone, and submandibular space become inflamed. Taking antibiotics will not solve the problem as the cause of inflammation is not eliminated. It is necessary to make an immediate appointment with a dentist to avoid even more serious complications.
Diagnosis of pericoronitis at iStomatolog dental clinic
