How are teeth extracted?
Extraction of teeth, also known as dental extraction or tooth removal, is a surgical procedure in dentistry that involves removing a tooth from its socket in the alveolar bone. It is commonly referred to as "pulling a tooth" by patients and is derived from the
Latin word "extraho," meaning "to extract or remove." At iStomatolog, we try to preserve all of the patient's teeth, even those that the patients themselves believe need to be removed. However, sometimes it is necessary to sacrifice a tooth and perform its extraction.
When is tooth extraction necessary?
Tooth extraction, like any surgical procedure, can be classified as either emergency (as soon as possible to prevent the spread of infection throughout the body) or planned.
Emergency reasons for tooth extraction include various inflammatory diseases that can lead to serious purulent infections (phlegmons, mediastinitis), transverse and longitudinal fractures of the tooth root, diseases of the periodontal tissues (when the teeth fall out of the oral cavity on their own), and others.
Planned tooth extraction may be necessary due to unsuccessful endodontic treatment (inability to pass through the channels of the tooth), orthopedic or orthodontic indications (when a tooth grows not in the dental arch and hinders rational prosthetics of the dental row, or when wisdom teeth are abnormally positioned).
Contraindications for tooth extraction
There are no absolute contraindications for tooth extraction, but there are delayed planned extractions. This is when the patient currently has infectious or cardiovascular diseases, decreased blood clotting (meaning they take medications to prevent high blood pressure), pregnancy in the early or late stages, and also the removal of milk teeth is contraindicated in children if there are no permanent tooth buds.
To ensure a smooth and painless tooth extraction, it is necessary to strictly follow the doctor's recommendations.

What are the types of tooth extractions?
Typical
The tooth is erupted and located in the dental arch. The roots are not significantly curved and do not communicate with the maxillary sinus or mandibular canal, and a periodontal gap is visible on X-ray imaging. Minimal surgical instruments are used for such extractions, and stitching is often unnecessary. The risk of complications during and after the procedure is minimal. The patient experiences no or minor pain after the extraction, which does not affect their quality of life. The use of painkillers is usually not necessary.
Unusual
When a tooth is located unusually (incorrectly): it has not erupted and lies in the jaw; the roots are intertwined with the roots of the adjacent tooth; the roots are located in the maxillary sinus or mandibular canal, etc. In such situations, tooth extraction is associated with risks and complications, requires the use of special tools and equipment with subsequent administration of medication, bone substitute, and suturing of the socket. If the patient follows all of the doctor's recommendations during the postoperative period, the risk of complications is minimized. Pain sensations during atypical extraction are more pronounced, and the use of pain relievers is required in the first few days.
What problems can occur after tooth extraction?
Usually, when a full medical history is taken and the patient undergoes necessary CT scans, complications are minimized. Side effects after tooth extraction may include swelling at the extraction site (which may sometimes be visible on the face), painful and uncomfortable sensations at the extracted tooth site, limited ability to open the mouth, and increased sensitivity to irritants near adjacent teeth. These symptoms typically resolve on their own and do not require additional treatment.
If complications develop after tooth extraction, it is necessary to seek medical attention. The most common complications are alveolitis (inflammation of the socket of the extracted tooth) and bleeding from the socket after the tooth has been removed. Typically, if bleeding persists in the first few days after extraction, alveolitis may develop. Other possible complications include nasal congestion and discharge (if the tooth roots were located in the maxillary sinus), numbness of the tongue, lips, or chin (due to injuries to the inferior alveolar nerve or compression caused by hematoma).
Rare complications include trauma to nearby teeth and dislocation of the jaw during extraction.
Basic recommendations for the healing period
After tooth extraction, we recommend refraining from physical exertion for several days, avoiding saunas, hot baths, and hypothermia. Oral hygiene during the healing period should be very thorough. In some cases, medications may be prescribed, such as pain relievers, anti-inflammatory drugs, antibiotics, and others.
What to do after tooth extraction?
After tooth extraction, we recommend options for tooth prosthetics or implantation. If a patient loses a front tooth, we usually install a temporary prosthesis during the rehabilitation period, thereby reducing the patient's aesthetic discomfort.

Reviews about iStomatolog
