Gingivitis treatment

What is gingivitis?

Gingivitis - is an isolated inflammation of the gums that does not affect the bone tissue of the jaws. The inflammation can affect the tissues around one or several teeth, for example, due to injury or overhanging edges of fillings and crowns, or it can affect around all teeth, such as in viral infections or in pregnant women with gingivitis.

As the pathology worsens, the inflamed gums become loose and rough, which causes a loss of tight contact between the tissues and the tooth crowns. Tooth loss due to inflammatory processes in the gums and periodontium is more common in adults than loss due to carious lesions. How to treat gingivitis? This is decided by a doctor after diagnostics and determination of the form and stage of the disease.

Symptoms reported by patients with gingivitis

  • Swelling
  • Redness
  • Unpleasant, sometimes even painful sensations
  • Bleeding
  • Unpleasant breath odor

Causes of gingivitis

There are many factors that can cause the appearance of gingivitis, below are the most common causes of gum inflammation.

  • Unsatisfactory oral hygiene
  • Poor quality restorations, crowns, and prostheses
  • Traumatic damage to the mucous membrane
  • Use of hormonal drugs
  • Viral diseases
  • In oncological patients after chemotherapy and radiation therapy.

Main principles of gingivitis treatment

  • limination of the cause - replacement of poor-quality restorations and crowns
  • Professional teeth cleaning, including ultrasonic removal of dental plaque and cleaning of hard and soft dental deposits
  • Reduction of inflammation
  • Stimulation of gum mucosa restoration
  • Selection of products for home care

In cases where gingivitis is caused by the use of medications, it is necessary to change the medication if possible. Gingivitis accompanying viral diseases does not require special treatment, except for thorough oral hygiene to prevent the attachment of bacterial infections. The same approach is used for gingivitis in pregnant women and cancer patients.

The cornerstone of gum inflammation treatment is the elimination of the etiological factor (cause).

Most effective methods of treating gingivitis

  • Laser - allows sterilizing the gum surface completely in one procedure
  • Ozone - initiates accelerated mucosa regeneration
  • Japanese gel preparation IG - prevents re-infection

Classification and stages of gingivitis development

Gingivitis can occur in acute or chronic form. In the first case, unpleasant symptoms (pain, swelling, redness) are pronounced and cause noticeable discomfort. Chronic gingivitis is accompanied by an unpleasant odor from the mouth and periodic exacerbation of pain in the affected area. The gum margin noticeably thickens and takes on the shape of a convex roller.

Chronic gingivitis is divided into several types, depending on the nature of the pathology and the severity of the inflammatory process. Gingivitis can occur in two forms depending on the spread of the disease. If the disease affects a small area of the gums, gingivitis is called localized. If the affected area covers the gums in the area of all teeth, then it is called generalized gingivitis. Based on the severity of the pathology, there are:

    • Mild and moderate forms - the inflammatory process affects small areas between the teeth (gum papillae);
    • Moderate severity - inflammation spreads to the gum margin;
    • Severe form - all soft tissues adjacent to the lower part of the teeth are inflamed.

Based on the degree of structural tissue damage, there are three forms of the disease: catarrhal, hypertrophic, and ulcerative. In the first case, the mucous membrane becomes brightly red, it swells, and there is abundant exudate secretion. With hypertrophic form, the size of the papillae between the teeth increases, the gums bleed during eating and toothbrushing, and deep gum pockets form.

Ulcerative gingivitis is an inflammatory process accompanied by ulceration and necrosis of the tissue at the gum margin. Separate treatment schemes are developed for each form of the disease.

    Complications of gingivitis

In a normally functioning immune system, local gingivitis resolves on its own without special treatment. However, inflamed gums are a source of infection that rapidly spreads in the oral cavity and affects the periodontal tissues, including the jawbone. Bacteria that penetrate the space between the loose gums and the tooth trigger the development of subgingival caries.

In the most severe cases, when the inflammatory process spreads to the bone tissue, various types of bone inflammation develop, accompanied by general intoxication, weakness, and an increase in body temperature. In the absence of urgent medical care, the progressing disease can turn into a purulent form, which is a life-threatening condition.

If the infection enters the bloodstream, complications from the ENT organs often arise (otitis, sinusitis, tonsillitis). If the infectious agent is streptococcus, damage to any organ or functional system of the body can occur. The weaker the immune system, the greater the likelihood of complications.

    How to identify gingivitis

Before developing a treatment plan for gingivitis, a doctor collects information about the patient's health status and listens to complaints. The specialist should know how long ago the first symptoms appeared, whether there has been previous treatment for gingivitis, and whether any complications have been identified. It is important to obtain information about the presence of chronic diseases (if any), the state of immunity, previous surgeries and injuries. The above information affects the choice of treatment tactics and medical preparations.

During the examination, a dentist easily finds the affected area and assesses the severity of the pathology based on visual signs. Very often, the inflammatory process occurs with constant trauma to the gum by the sharp edge of the crown or a poor-quality filling. The doctor eliminates the traumatic factor and prescribes drug therapy. In this case, the disease resolves without consequences.

If there are reasons to believe that the inflammatory process has spread beyond the gum, the dentist orders additional tests (mainly X-rays and CT scans) to determine the condition of the roots, periodontium, the degree of destruction of the alveolar processes and jaws' bone tissue.

Additional diagnostics are required in those cases when the disease needs to be differentiated from pathologies with similar clinical symptoms, including gum fibromatosis, mild chronic periodontitis, infiltration of the gum in blood diseases, and others.

    Treatment of gingivitis

The main objectives in treating gingivitis are:

    • Eliminating the cause - replacing poor quality restorations and crowns
    • Professional teeth cleaning, including ultrasonic removal of dental calculus and cleaning of hard and soft dental plaque
    • Reducing inflammation
    • Stimulating the recovery of the mucous membrane of the gums
    • Selection of products for home care.

In cases where gingivitis is caused by medication, if possible, the medication should be changed. Gingivitis caused by viral diseases does not require special treatment, except for thorough oral hygiene to prevent the attachment of bacterial infection. The same tactics are used in the treatment of pregnant women and cancer patients.

The fundamental principle in treating gum inflammation is to eliminate the etiological factor (the cause).

Methods for treating gingivitis at iStomatolog in Kyiv

The laser allows the entire surface of the gums to be completely sterilized in one procedure. Prior to the procedure, the tooth is cleaned using ultrasonic cleaning to remove plaque and dental calculus. The procedure is painless and does not damage the enamel. Laser sterilization involves precise laser beam exposure to the affected area.

With laser therapy, foci of harmful microbes are destroyed, while healthy tissues are not damaged. The laser completely removes bacterial plaque from the surface of the gums, promotes increased local blood flow, and reduces the severity of the inflammatory process.

The type of laser and radiation settings for treating gingivitis are selected by the dentist, taking into account the form, nature, and scale of the disease. Laser therapy does not cause complications with qualified treatment and adherence to the dentist's prescriptions after the procedure.

Ozone enhances tissue regeneration. The method of ozone treatment of the affected area on the Heal Ozone device is used as an independent method of treating gingivitis, as well as in combination with other types of therapy. Ozone treatment produces the following results:

    • Reducing swelling of the mucous membrane and soft tissues

    • Reducing the number of dilated blood vessels

    • Suppression of the inflammatory process

    • Initiation of intensified regeneration of gum and bone tissue.

Ozone enhances blood circulation, destroys harmful bacteria, which leads to rapid healing of injuries and wounds in areas of inflammation.

The Japanese gel preparation IG prevents reinfection. The gel film forms a protective barrier on the gums, providing an anti-inflammatory, soothing, and bactericidal effect.

Antibiotics for treating gingivitis are only prescribed in advanced cases, when a purulent process is detected in periodontal pockets, extensive ulcers, jawbone inflammations, and abscesses. Proper oral care after completing the treatment course is a guarantee of preventing recurrence, reliable protection of teeth and soft tissues from infection. If necessary, drugs for restoring gum tissue are prescribed. The dentists at our clinic will develop a personalized teeth care plan, taking into account the nature and scale of existing problems.

    FAQ

If you have noticed one or more of the following signs, it is a reason to seek a consultation with a periodontist or at least your treating dentist: bleeding gums (or around several teeth) during brushing, eating solid food; tooth mobility; root exposure; defects on teeth at the gum line; unpleasant odor from the mouth, despite careful and regular hygiene; redness and swelling of the gums; unpleasant sensations of soreness, itching in the jaws; increased tooth wear; displacement or tilting of teeth, changes in bite.
Gum treatment is carried out comprehensively with the participation of several specialists, not just a periodontist. There are many reasons that contribute to problems with the gums, such as: poor quality fillings and crowns with overhanging edges; missing teeth and overloading of the remaining teeth; poor hygiene; incorrect bite and more. Therefore, competent cooperation between the dentist, orthopedist, surgeon, orthodontist, and periodontist is fundamental for a successful result. Gum treatment begins with the professional removal of dental calculus and plaque, radiographic examination and diagnosis with determination of the degree of the disease, after which an individual treatment plan is developed. The most effective methods for treating gums are: vector therapy; laser therapy; ozone therapy; curettage; gingivoplasty.
With the presence of X-ray examination and professional teeth cleaning, the examination takes about 30-60 minutes. The periodontist determines the nature of the disease, the degree of bone loss, the depth of periodontal pockets, and, in case of severe pus discharge, takes a smear from the periodontal pocket to determine the type of microorganisms and their sensitivity to medication. In addition, the periodontist determines the need to consult with a related specialist (therapist, orthodontist, orthopedist, or surgeon). All manipulations are absolutely painless.
Unfortunately, it is not possible to recognize gum disease independently. The diagnosis is made based on the patient's complaints, clinical signs, and analysis of X-ray examinations. However, the presence of one or several signs (see "When do you need to go to a periodontist?") can be a reason to consult a periodontist.
The cost of a periodontist examination at the iStomatolog Center in Kyiv is 10 euros. If necessary, you can also receive an examination from a therapist, surgeon, and orthopedist with a detailed treatment plan and financial justification. The cost of a comprehensive consultation will be about 20-25 euros.

Nice clinic. Very attentive doctors, very nice people. Competent in their job. All procedures, implantation, crown installation in 1 day, filling - everything is absolutely performed according to the protocols. The clinic is clean, tea and coffee will be offered. Protection against covid is carried out - everyone is wearing masks, shoe covers, the temperature is measured. Thank you very much to Maxim Valerievich, Elena Dainetskaya, Lidia Vladimirovna. We wish you prosperity!
Excellent! I was limited in time and came here in hope to fix broken front tooth crown. This is the only available place that made it in one day. Wonderful friendly staff and fabulous technology. Great!
