What is parodontosis?
Parodontosis is a gum disease characterized by systemic damage to all tissues surrounding the tooth - gums, tooth ligament, and jawbone. The absence of inflammation is a characteristic difference from periodontitis.

Radiographically, periodontitis is characterized by uniform horizontal loss of bone tissue in the area of all teeth.

Causes of parodontosis
The development of parodontosis is directly related to the gradual pathological changes in blood vessels (the process of sclerosis), which leads to a narrowing of the lumen of capillaries and thickening and deformation of their walls. As the disease progresses, the amount of oxygen and nutrients necessary for maintaining the normal state of the periodontium decreases, which is the cause of tissue destruction around the teeth.
At the first stage, the bone tissue becomes sclerotic, then the soft tissues, gums, periodontal ligament, and alveolar bone undergo dystrophic changes. Fibrous fibers gradually replace the natural ligament of the tooth (periodontium), which leads to irreversible changes in the structure of the periodontium. The tooth becomes fused with the bone and loses its natural physiological mobility necessary for distributing pressure during chewing. As the tissue atrophy progresses, the risk of developing an inflammatory process increases due to increased stress on the teeth.
It is difficult to determine the main cause of the development of sclerotic tissue dystrophy, but medical science knows the main provoking factors that trigger the mechanism of parodontosis development. This list includes:
- chronic diseases of the vascular system (hypertension, atherosclerosis, vegetovascular dystonia);
- metabolic disorders (endocrine diseases leading to changes in hormonal balance);
- some diseases of the nervous system;
- long-term medication therapy;
- hereditary predisposition;
- avitaminosis;
- harmful effects of environmental factors (all types of radiation, polluted resources, work with poisons and toxic substances).
Most often, several factors unrelated to each other act as provokers of parodontosis development. For example, the presence of a genetic predisposition combined with smoking and a low immune status increases the likelihood of pathology several times. In any case, parodontosis is always associated with the overall health of a person, so the treatment of the disease should be systematic and comprehensive.

Classification and stages of parodontosis
Parodontosis - is a chronic disease that can be in a stage of active progression or remission, when the development of the pathology slows down or stops. The stages of the disease reflect its severity:
- First (mild) - symptoms are weak or absent, while destructive changes in bone tissue are already occurring (the pathology is visible on an X-ray).
- Second (moderate) - multiple dental deposits (calculus) are found during dental examination, exposed tooth necks and roots are visible, changes in the structure of interdental bone partitions (reduction in height) are clearly visible on an X-ray, increased tooth wear and slight mobility are noted. Patients complain of discomfort in the gum area, particularly during chewing, and sometimes of itching in the tissues around the teeth. The dental arch is deformed, acquiring a fan shape, wedge defects appear. Tooth sensitivity to thermal (cold and hot) and other irritants increases;
- Third degree (severe) - for this stage, tooth root exposure is 5 mm or more, destruction of two-thirds of the length of the roots is clearly visible on an X-ray. Teeth become loose, displaced, or tilted to the side, and can fall out.
Diagnosis of parodontosis
The main methods for determining the severity of parodontosis are visual examination and radiographic examination. Orthopantomography shows all dystrophic changes well and determines the degree of bone tissue loss. Additional tests may be prescribed to diagnose the functional state of the blood vessels in the periodontal tissues, to identify areas of the most pronounced sclerosis, and to determine the degree of tissue oxygen saturation, if necessary.
Parodontosis is a disease with pronounced specific signs, which allows for a quick and accurate diagnosis without special differentiation. The difference from periodontitis is the loss of bone tissue within the horizontal plane, characteristic changes in the appearance of teeth, the presence of wedge-shaped defects, increased tooth abrasion, and the absence of an inflammatory process.
However, the last statement cannot be considered 100% accurate. Failure to follow oral hygiene rules can lead to the development of inflammatory foci, bleeding and swelling in the gum area, characteristic of catarrhal gingivitis. The inflammatory process occurs against the background of the activity of pathogenic bacteria, which are part of dental calculus and soft plaque. After professional ultrasonic teeth cleaning, the inflammation gradually subsides, and symptoms of classic periodontitis return.
Treatment of parodontosis
Treatment of parodontosis is a complex multistage process based on an individual approach. The doctor must accurately determine the cause of the pathology in order to fundamentally eliminate it or mitigate the negative impact of the underlying disease on the dental tissues.
