Who is a candidate for All-on-4 implantation?
The absence of teeth is a significant aesthetic and functional defect that significantly reduces the quality of life and creates psychological discomfort. Removable dental constructions, which have been used for many years in cases of extensive or complete tooth loss, have numerous drawbacks, including a poorly fitting prosthesis that can shift with significant bone atrophy, restrictions related to consuming hard foods, and the need for replacement and correction every few years.
All-on-4 implantation is one option for prosthesis in cases of complete tooth loss in the upper or lower jaw (or both) that enables the creation of a comfortable, non-removable structure that meets aesthetic and functional requirements.
The Invention of the All-on-4 Technology
Interestingly, this technique is not innovative and was first used in 1998 by dentist Paulo Malo in Portugal. Successful treatment of the first patient spurred the development of this direction.
The Nobel Biocare company patented the technology of installing a non-removable dental prosthesis supported by four implants All-on-4. It has conducted the most clinical trials on the use of non-removable prosthesis on toothless jaws with immediate load.
For almost 20 years, patients who have lost all their teeth for various reasons have had the opportunity to install comfortable non-removable prostheses using the All-on-4 method and significantly improve their quality of life. The technology is well-studied, safe, and reliable.
Indications for All-on-4 implantation
Indications for prosthesis on 4 implants using the All-on-4 method are:
- Complete tooth loss;
- Partial tooth loss when the remaining teeth need to be removed for various reasons.
When planning All-on-4 prosthesis, it is important to note that all installed implants must be connected into a single structure with a special metal frame on which the dental prosthesis will be fixed.
This will provide the necessary strength for the entire structure.
Stages of All-on-4 prosthetic treatment
Stage 1: Diagnostic examination
Analysis of computer tomography data to determine the height, width, and density of the jawbone. Examination of the patient's oral cavity. An important criterion in selecting the "All-on-4" prosthesis is determining the position of the gum line in the edentulous jaw when smiling. Agreement on the future prosthesis design.Stage 2: Planning
Virtual placement of implants in the jaw with consideration of the planned prosthesis design. Making a template and temporary prosthesis (implants can be placed without using a template).Stage 3: Implantation and temporary prosthesis
Implant placement using a surgical template and fixation of a temporary acrylic prosthesis. The prosthesis can be fixed immediately after implant placement or after 2-3 days. Ceramic composite prostheses can be used as an adaptation prosthesis.Stage 4: Osseointegration and permanent prosthesis
Osseointegration of implants (adherence to the jawbone) for 4-6 months with subsequent replacement of the temporary prosthesis with a permanent one.The practical experience of the iStomatolog dental center shows that with clear implementation of implantation and prosthesis planning protocols, the success rate of the All-on-4 procedure is more than 98%.
The All-on-4 prosthetic principle
The name of the technique indicates that the foundation of the dental structure consists of four supporting elements, which are the implants - titanium rods inserted into the jawbone. The implants are placed in such a way as to provide even distribution of the load on the jaw and prevent possible displacement of the prosthesis in any direction.
How implants are placed according to the All-on-4 technique?
- Two implants are vertically and parallelly placed in the front area (area of the anterior teeth);
- One implant is placed at a 45-degree angle in each of the lateral areas of the jaw (in the area of the 5th and 6th molars).
Implant placement at an angle allows for increased contact area between the implant and the bone tissue during chewing.

Advantages of the "All on Four" implant method
The technique of placing dental prostheses on four implants is well-studied and has several valuable advantages compared to other tooth replacement technologies for full edentulism:
- Financial savings compared to classic implantation and prosthetic methods
- No need for bone grafting to increase the volume of bone tissue in both width and height, or sinus lifting
- Minimal trauma (implants are placed in a gentle way thanks to the use of 3D modeling technology and surgical templates)
- Possibility of installing the prosthesis immediately after implantation
- Fast tissue regeneration period
- High strength of the structure due to the consolidation of all implants into a single system with a titanium beam to which the prosthesis will be attached
- Short adaptation period (most patients get used to the prosthesis within 14 days)
- Possibility of adjustment (the prosthesis construction is fastened with screws, allowing the dentist to remove it without damaging its integrity).
Chewing load is recommended from the first days after implantation - this is necessary for stimulating blood circulation and bone tissue regeneration. The hardness of food is gradually increased, and accordingly the load on the implants gradually increases.
What type of prosthesis is used in All on 4 implant placement?
The standard option for prosthetic replacement on four implants involves the installation of acrylic prostheses (plastic), secured on a titanium frame. The base of the structure imitates a part of the gum and corrects irregularities of the alveolar ridge. Materials such as ceramic composite, zirconia dioxide, and ceramics can be used to create permanent prostheses, which are characterized by a long service life, high biological compatibility, and superior aesthetics.
What implants are used for the All on 4 technique?
Most globally recognized implant manufacturers (Neobiotech, Straumann, Megagen, MIS, Dentis, and others) have implant and multi-unit lines in their arsenal for implantation and prosthetics of edentulous jaws. However, the leader and founder of this type of edentulous jaw prosthetics is Nobel.
Disadvantages of the All on 4 system
When evaluating the possibility of using the All on 4 method, fairly high requirements are imposed on the condition of the jawbone tissue:
- the height of the bone in the implant installation areas on the upper jaw is at least 10 mm, on the lower jaw - 8 mm;
- the width of the alveolar ridge is at least 5 mm;
- high bone density;
- easy or moderate degree of bone tissue atrophy.
With severe atrophy or "soft" bone, implantation and prosthetics with the All on 4 system are not performed. When prosthetic with a non-removable structure, the possible number of teeth on the prosthesis for one jaw is 10-12 units. Unfortunately, this is often not enough to create a beautiful smile line.
What prostheses are used in All on 4 implant prosthetics?
Prostheses can be conditionally removable and non-removable.
Conditionally removable prosthesis: replaces up to 14 missing teeth. Provides full chewing efficiency and high aesthetics. The disadvantage is the need to remove and clean this prosthesis.
Non-removable prostheses do not need to be removed, but they are limited in the number of installed teeth to 10-12 for one jaw. This is not enough for a wide smile of the patient, and in such a situation, prosthetics with "All on 6" are offered, which provides a greater number of supports for the prosthesis and the possibility of making a longer dental arch.
Adaptation and care after implantation and prosthetics of "All on 4"
Adaptation to the prosthesis takes place within 1-2 weeks. After this time, the prosthesis is already perceived as one's own teeth, speech normalizes, the patient no longer focuses attention on the prosthesis when eating, and the fear of possible trauma to the prosthesis/implants/jaw disappears.
The "All on 4" design does not require special care. It is enough to clean it with a toothbrush and paste twice a day, and undergo professional hygiene and examination by a dentist once every six months.

Reviews about iStomatolog
