Alignment of teeth with metal braces
The parents of a young man came to the iStomatolog clinic with complaints about the unsatisfactory aesthetic appearance of his smile (crooked teeth), as well as the inability to properly bite food due to the lack of contact between the front teeth and excessive protrusion of the lower jaw forward. The patient wished to align his teeth, bring all teeth into contact for proper chewing, and also correct the shifted central line along the incisors of the upper and lower jaws, as this defect visually made his face asymmetric.
The treatment of this clinical case presented a complex challenge, as the patient had a number of serious problems, including:
- Protruded lower jaw due to facial skeletal features (excessive growth of the lower jaw and mesial occlusion)
- Open bite in the anterior region (lack of contact between the front teeth)
- Crossbite on the right, crowding of the teeth.
In addition, there was pronounced facial asymmetry and a shifted central line. Treating the above pathologies in such situations usually requires not only the installation of a braces system, but also orthognathic surgery on the jaws. Our clinic's doctors have techniques that, in most cases, allow for avoiding such surgical interventions.

The patient underwent an X-ray examination, digital 3D models of the jaws were created, detailed measurements were taken, and the final result was planned.

The proposed treatment plan for the patient included the installation of braces, as well as preparation of the oral cavity through professional hygiene cleaning and treatment of caries.
3 Stage
BRACES FIXATION
The orthodontist decided to fix a ligature metal brace system on the upper and lower jaw in one visit.
4 Stage
WISDOM TOOTH REMOVAL
The removal of the lower 8th tooth was planned for 3 months after the braces were fixed because it obstructed the movement of the lower jaw's teeth. The decision was made to leave the upper 8th teeth because they did not affect the treatment result, and the patient could remove them at any other time if desired.
5 Stage
BRACES REMOVAL
The patient's wishes were fully met: the alignment of the teeth was normalized, chewing food was no longer difficult. The central line of the front teeth coincides, the face looks symmetrical, and the smile has a pleasant appearance. In this case, surgical intervention (orthognathic surgery) was avoided. A positive result was achieved by using a standard metal brace system with the patient wearing elastic traction (elastics) continuously. The treatment period was 16 months. To fully stabilize the position of the teeth and prevent relapse (repeated tooth misalignment), the patient was recommended to wear retainers for the next 3-5 years.

Metal braces can be vestibular (attached to the outside of the teeth) and lingual (installed on the underside of the tongue). According to the design, there are ligature and self-ligating systems. Different alloys based on titanium and steel are used in orthodontics.
The brace system is an individual construction. A bracket is glued to each tooth, and then a wire is pulled through, which is secured with ligatures or special fixatives.
The main component of the system is the force wire, which sets the vector of tooth movement. As their position changes, the wires are replaced with stiffer ones until the teeth are in the correct position.
