Main components of quality dental implantation
The concept of "quality implantation" includes not only the installation of the implant itself, but also the ability to install crowns on the implants to achieve a good aesthetic and functional result. The implant is essentially the foundation for the future crown. The end result of implantation is not the implant itself, but the complete replacement of the missing tooth.
The 1 stage of quality implantation is planning
Planning is carried out jointly by the surgeon and the prosthetist. Using CT scans and 3D models of the jaws, the following is determined:
• the final construction of the orthopedic structure (crowns)
• the necessary number and location of implants
• determination of the type of bone (by its density), height, width, location of important anatomical formations
• selection of the optimal implant system, their length and diameter
• the need for additional surgical procedures such as sinus lifting and bone grafting.
The 2 stage of quality implantation is oral cavity examination
• determination of the hygiene index
• the presence of inflammatory diseases of soft tissues (gums and oral mucosa)
• the presence of inflammatory diseases of bone tissue (tooth cysts, periodontitis)
• the presence of foci of chronic infection (caries teeth)
The 3 stage of quality implantation is patient history
• the presence of absolute contraindications for implantation
• the presence of relative contraindications for implantation
The 4 stage of high-quality implantation - patient preparation
• professional dental hygiene
• treatment of teeth (if necessary)
• treatment of gums (if necessary)
• consultation with related specialists and blood tests (if the patient is under the care of a doctor for any illness)
The 5 stage of high-quality implantation - implant placement
• preoperative preparation
• implant placement
• medication management after implantation
• recommendations for the post-implantation period
The need for bone grafting and sinus lifting:
This is determined individually for each clinical case after analyzing the data from a jaw CT scan. The indication for the procedure is a lack of bone for implant placement, both in terms of width and height. Often, bone deficiency is determined after prolonged tooth loss, wearing removable dentures, or poorly made constructions that have caused chronic inflammatory prosthesis, or with gum diseases (periodontitis or periodontosis). Modern implant systems offer a huge selection, and often it is possible to do without bone grafting simply by choosing the optimal implant system.
A properly placed implant should have primary stability, that is, be firmly installed in the bone and be completely within the jawbone; the mucous membrane above the implant should be sewn up.
The 6 stage of high-quality implantation - post-implantation period
At this stage, osseointegration of the implant (implant fusion) occurs:
• examination of the patient the next day or the day after the tooth implantation
• removal of stitches on day 7-10
The patient needs to carefully monitor oral hygiene and follow the doctor's recommendations (taking medication according to an individual schedule).
Implant fusion occurs within 1 to 6 months.
The 7 stage of high-quality dental implantation
• placement of gum formers
• 3D scanning of the jaw
• production of individual abutment and crown for the implant
There are different options for dental implantation and the cost depends on various factors. For example:
Example 1: Functional implantation
When a lower molar tooth is missing and the patient needs to be able to chew properly, aesthetics are less important. In this case, a regular functional implantation is possible without bone grafting if there is minimal bone deficiency of 2-4 mm after tooth extraction.
Example 2: Functional-aesthetic implantation
When upper front teeth are missing and the patient wants a natural-looking replacement, bone deficiency of 2-4 mm can be corrected with a bone graft at the same time as implant placement to achieve both functional and aesthetic goals.
Example 3: Aesthetic implantation
When a front tooth is missing, bone grafting is usually necessary. After 4 months of healing, the implant can be placed if there is enough bone tissue. If not, additional bone grafting is needed. After the implant is integrated, the condition of the bone and gums is evaluated, and if necessary, gum surgery is performed before the permanent crown is placed.
It is important to note that the success of dental implantation depends on accurate diagnosis, planning, preparation, proper implantation technique, and following the doctor's instructions during the post-operative period.
